A) Start your online application. You must create an online account to use the Online Licensing System.

After submitting your application the Dental Board will email you a date and time to have fingerprints taken at an office called: IdentoGo, so check your email.
B) Once you get their email go to IdentoGo and mail them a copy of your IdentoGo receipt along with your Self-Query Report. (Never open the Self-Query Report, it must remain sealed). Mail both documents to the following address:
